In some cases, you may need additional support after you are discharged from the hospital.
Your care team can guide you regarding:
- home care
- home blood testing of your newborn for bilirubin
- dedicated nursing care
- physical therapy or rehabilitation
- medical equipment or assistive devices
- 24-hour telephone support for issues related to breastfeeding
- other services that may be needed for your safe recovery
Follow-up Appointments
Follow-up care after discharge is an important part of your treatment.
Before you leave, make sure you know:
- when you need to be re-evaluated
- which doctor to contact
- how to schedule or confirm your next appointment
Transfer to Another Healthcare Provider
If you need to continue your care at another healthcare facility or rehabilitation center, your care team will coordinate the transfer and forward the necessary medical information to ensure a smooth transition in your care.
