Your Discharge from the Hospital

Planning for your discharge begins at the start of your hospitalization and is part of your personalized care plan. Your attending physician assesses your needs and plans your safe transition from the hospital to your home or to another healthcare facility, when necessary.

During your hospital stay, you will discuss matters related to your medication, follow-up care after discharge, the need for further tests or follow-up visits, as well as any support services you may need after you leave.

Our goal is to ensure that, by the day of your discharge, you have received all the necessary information and instructions so that you feel safe and adequately prepared to continue your care.
